- every policy in the state file: carrier, effective and expiration dates, cancellation or non-renewal, reinstatement, employee-leasing arrangements
- in Texas, every non-subscriber filing (DWC Form-005): period, date received, locations covered
- the status on the file date, and the date that matters to you if you give it
- what was not found, written down: the files checked and the empty results
- the exact citation: state agency, file, date, portal

What the report is NOT
It is not a certificate of insurance, not legal advice and not a state document. It states what the state files show for this employer on their date. An absence from a file is not proof of no coverage.
